The 2020 PAS Congress Fellowship Scholarship will provide awarded applicants with waived registration fees and $1,000 USD to assist with the cost of travel and participation at the 2020 PAS Congress. Please note that housing, meals and transportation are not provided.
Only one scholarship will be awarded per individual. Applicants that applied for and are awarded a 2020 PAS Congress Travel Grant Award will not be eligible to receive the Fellowship Scholarship Award. Details regarding the 2020 PAS Congress Travel Grant Award can be found here.
Submitted 2020 PAS Congress Fellowship Scholarship applications received a cumulative score based on the responses provided. Fellowship verification letters were reviewed and program directors were contacted as needed for confirmation. The selected Fellowship Scholarship awardees will be verified by the PAS Congress Scientific Program Committee Chairs.
All applicants were sent an email notification regarding the outcome of their application in early December 2019. The results of the notification are final; requests for reevaluation will not considered.

If you are awarded a Fellowship Scholarship and your attendance status for the 2020 PAS Congress changes for any reason, please contact the MDS International Secretariat at email@example.com at the earliest opportunity so we may redistribute the scholarship to another qualified candidate.
Please note: the 2020 PAS Congress Fellowship Scholarship is only transferrable via the MDS International Secretariat.

W-8 / W-9 Form

NON-AMERICAN FELLOWSHIP SCHOLARSHIP RECIPIENT
All non-American individuals receiving money from MDS for work done outside of the United States are required to complete a W-8 form for the Society to keep on file.
This form is simply to state that the recipients are not citizens of the United States, and unless the recipients were to become citizens of the United States, this form would only need to be completed once. The Society is not required to withhold tax on payments to non-American individuals and entities for work done outside of the USA and the W-8 form provides the Society with proof of an individual’s citizenship. Payment cannot be issued if MDS does not have a copy of your W-8 on file.
AMERICAN FELLOWSHIP SCHOLARSHIP RECIPIENT
All American individuals receiving money from the Society must complete a W-9 form. Payment cannot be issued if MDS does not have a copy of your W-9 on file.
DUAL CITIZENSHIP (AMERICAN) FELLOWSHIP SCHOLARSHIP RECIPIENT
If you have dual citizenship (American citizenship plus any other country), please complete the W-9 Form.

Fellowship Scholarship claims filed prior February 21, 2020 will be processed within 45-days following the 2020 PAS Congress. Fellowship Scholarship claims filed after February 21, 2020 will be processed by the MDS International Secretariat within 45-days of the receiving the completed claim forms.
Fellowship Scholarship claims must be filed within the same calendar-year as the 2020 PAS Congress. Claims for the 2020 PAS Congress must be received by June 1, 2020.
